The Treatment of Osteoporosis, focused on Bisphosphonates

Abstract
Osteoporosis and related fractures are one of common disease for geriatric rehabilitation, also one of important cause of impairments. Bisphosphonates have been most commonly used for treatment of osteoporosis. Bisphosphonates have been proved their efficiency for increment of bone mineral density, risk reduction of vertebra bone and those of non-vertebra bone. However, long-term use of bisphosphonates has also been proved the relation with complications, such as medication-related osteonecrosis of jaws, or atypical femur fracture. Current a clinical guideline proposed treatment duration of 5 years for Alendronate and Risedronate, 3 years for Zoledronate. However, the evidences for proper duration of treatment, or prevention of complication related with long term use, are limited to uncover. Thus, clinician should decide drug holiday for patients with osteoporosis, with individualized and empirical bases. Understanding and proper medication of bisphosphonates would be one of most important therapeutic strategies for patients with osteoporosis.
